The Space Between: Implementing Spiritual Care in Community Aged Care supports organisations in planning, implementing and evaluating community based spiritual care. In this program, community dwelling older adults receive the short term support of specialist spiritual care professionals and/or spiritual care volunteers over the course of 6-12 fortnightly sessions.
This guide supports the development, implementation and evaluation of the program, including workforce training requirements, referral processes, engagement with the multi-disciplinary team, spiritual assessment and review processes.
Some of this guide had its origins in materials developed by Catholic Healthcare for use in their community aged care services. Catholic Healthcare won the 2018 Meaningful Ageing Australia Quality Practice in Pastoral and Spiritual Care Awards. Our gratitude to Catholic Healthcare for sharing this resource with our members and to Prime Super for supporting the awards, and the publication.
“It helps me to believe in myself and to be in touch with a healthy part of myself, and not be so negative… and to give me hope every day on this journey because it is hard. And it is easy to lose hope. So it brings light into dark places in your life.”
